I'm the publisher of Co-Kinetic Journal — a peer-reviewed clinical journal for physiotherapists. I started in 1999 with Sportex Medicine, which evolved into Co-Kinetic. Over 25 years, I published 100 issues of clinical education content, patient-facing resources, and evidence-based research.

I've always been a bit of a closet geek — I've been building software systems throughout my career, long before AI made it fashionable. We started in 1999 with Sportex Medicine — a peer-reviewed clinical journal for physiotherapists. The ethos was simple: clinical content doesn’t need to be dull. Honestly, this is where I'd encourage you to take a hard look at what's happening. Nearly half of all people now use AI search as their primary way of finding information. If you're not investing in content, you're not just missing out on AI visibility, you're at risk of disappearing from search altogether. Word of mouth is valuable, but it has limits. This is a highly competitive market, and trust is built online now. If patients can't find you, or if they find your competitors first, they'll go elsewhere. The numbers don't lie, and the trajectory is clear. You don't need to spend a fortune, but doing nothing is no longer a neutral choice.
Some of the quick wins we share can have an effect within days, because AI tools re-crawl and re-evaluate content regularly. But real, sustained AI visibility compounds over time. We have seen clinics achieve significant, meaningful results within three months of applying this framework consistently. There are no guarantees, because this is a fast-moving space and the AI platforms themselves are constantly evolving. But the direction of travel is clear, and starting now will always be better than starting later.
It applies to every size of clinic, and in some ways smaller clinics have an advantage. AI tools don't rank websites by size or budget. They recommend content they trust. A solo practitioner with well-structured, clinically credible content can outperform a large multi-site practice with a poorly optimised website. We've seen it in our own data. In fact, some of the best AI citation rates we've recorded belong to smaller clinics with clear, specific, authoritative content. If anything, this is an opportunity to compete on quality rather than spend.
Sort the content first. This is one of the most common mistakes we see. Clinics spend months and significant money on a new website design, then wonder why nothing has changed. The reason is that AI tools, and search engines, don't see your design. They read what's underneath it. The substance. A beautifully designed website with thin, generic content will be invisible to AI. A straightforward website with well-structured, clinically credible, specific content will get recommended. Your website does need to do its job, and conversion rate optimisation matters once people arrive. But if you're not getting found in the first place, the design is irrelevant. Get the content right first. Build the authority. Then redesign around it if you want to. Content also compounds over time, so the sooner it's live, the sooner it starts working for you.
This is something most SEO agencies don't fully understand, and it matters enormously. Healthcare content falls into what Google and AI platforms classify as YMYL, Your Money or Your Life. This is content that could directly affect someone's health, safety, or financial wellbeing. AI tools hold YMYL content to a significantly higher standard than general web content. They look for clinical credentials, author qualifications, evidence-based references, professional affiliations, and demonstrated expertise. A generic content writer producing keyword-optimised blog posts won't pass that bar. Neither will AI-generated content that hasn't been properly structured and validated. The rules are genuinely different, and if the person writing your content doesn't understand this, you won't just get lower visibility. You risk being actively deprioritised. This is one of the reasons clinical credibility is at the core of everything we do at Co-Kinetic.
Almost certainly more so. Specialised clinics are actually in a stronger position when it comes to AI visibility, because AI tools are very good at matching specific patient queries to specific expertise. If someone searches for a sports physio who works with runners, or a clinic that specialises in post-surgical rehab, a well-optimised specialist clinic will consistently outperform a generalist one. The more clearly and specifically you can articulate what you do and who you help, the more confidently AI tools will recommend you. Niche is not a disadvantage here. It is an asset.
